How cholesterol ratios are calculated

Interpretation bands

These ranges are a quick way to read the calculator output. They are not a diagnosis, and they should not override the LDL, non-HDL cholesterol, triglyceride level or 10-year cardiovascular risk estimate used by your clinician.

Triglyceride/HDL can point toward insulin-resistance patterns in some groups, but it is not a diagnostic test and cutoffs vary by population. Treat it as a conversation starter, especially if triglycerides are high or HDL is low.

Why ratios add context

An LDL value of 130 mg/dL can carry different meaning when HDL is 30 mg/dL (LDL/HDL 4.3) than when HDL is 80 mg/dL (LDL/HDL 1.6). Ratios capture that balance. Still, modern cardiovascular prevention usually focuses on LDL cholesterol, non-HDL cholesterol, triglycerides and overall risk factors rather than a ratio alone.

Units

Use the unit printed on your report. Many US reports use mg/dL; many European and Commonwealth reports use mmol/L. Conversion:

  • Cholesterol: 1 mmol/L ≈ 38.67 mg/dL
  • Triglycerides: 1 mmol/L ≈ 88.57 mg/dL

The calculator converts automatically. Ratios are unit-free as long as the values are entered in one consistent unit system.

What can move these numbers

  • HDL support: regular physical activity, stopping smoking and weight management where needed. Do not start drinking alcohol to raise HDL.
  • LDL lowering: reducing saturated and trans fats, increasing soluble fibre, using plant sterols or stanols, weight management and medicines such as statins when indicated.
  • Triglyceride lowering: reducing refined carbohydrates, sugary drinks and excess alcohol, increasing activity, weight management and prescription omega-3 or other medicines when appropriate.

Lifestyle changes often take several weeks to months to show on a lipid panel. Your clinician can advise when to repeat testing.

Not medical advice

A ratio in a higher band does not predict an event next week. It is one input among many: age, sex, blood pressure, smoking, diabetes, family history, kidney disease, pregnancy history, inflammatory conditions, medicines and sometimes calcium scoring or ApoB/Lp(a) testing. This tool helps you read your report; it does not replace medical interpretation.

Frequently Asked Questions

Many lipid panels can be done fasting or non-fasting, depending on the reason for testing. Triglycerides are the most meal-sensitive value, so your clinician may request an 8-12 hour fast if triglycerides have been high, if a calculated LDL needs confirmation, or if your local lab requires it.

Both can be useful, but treatment decisions usually start with LDL, non-HDL cholesterol and your overall cardiovascular risk. Ratios can add context when HDL is very low, triglycerides are high or metabolic syndrome is suspected.

Many labs estimate LDL from total cholesterol, HDL and triglycerides. The older Friedewald equation can be less accurate when triglycerides are high or LDL is very low. Some labs use newer equations or direct LDL measurement, and your clinician can decide when confirmation is needed.

The calculator is stateless — nothing is saved. Copy values into a personal health journal or spreadsheet to track trends. Single results can vary with illness, recent meals, medicines and lab methods, so look for consistent direction across repeat tests.
