A birth plan or preferences worksheet can make it easier to explain what matters to you and which questions you want to discuss. It is not a contract or a prediction. The ACOG sample birth plan describes a plan as a way to communicate wishes and notes that it cannot guarantee how birth will unfold. The NHS guidance similarly advises discussing choices with a midwife and recognising that plans can change.
This tool intentionally gives you blank, editable fields instead of telling you what to choose. You may use them to record a communication preference, a comfort or environment topic, a question about labour or birth, or a post-birth and newborn-care question. Review the completed wording with your maternity care team well before the due period, then update it if the team explains a local policy, availability issue, or clinical reason for a change.

Respectful care includes clear communication, privacy, and involvement in decisions. The World Health Organization describes individualized, supportive care and good communication as important parts of a positive childbirth experience. This worksheet supports that conversation, but it does not replace it.
Keep the wording practical and specific to you. Avoid putting private information in a shareable copy unless you are comfortable sharing it.
